I understand that classes will be held at The Well. Mrs. Hart will lead all instruction, assign weekly assignments, tests, and projects. She will grade the work and provide me with a class grade that I can use for my records. It is up to me to hold my child accountable for work completion. I understand that I may be provided worksheets that I may need to print.

I understand that spots are limited and that my student is not fully enrolled until my payment is submitted. I understand that students must have good and punctual attendance.

I would like to pay in full. At the time of registration, I will send the entire amount of $300.

I would like to pay in 3 installments. I understand that there will be an added $20 payment plan fee. At the time of registration, I will pay that $20 fee as well as $100 toward the standard tuition. My second payment of $100 will be due by 10/4, and my final $100 will be due on 11/1. I understand that if I do not pay all payments on time, my child will not be able to participate.
